Understanding oot — out of trend
OOS and OOT answer different questions. OOS asks whether this result passes. OOT asks whether this result belongs to the same population as the previous ones. A result can sit comfortably inside specification and still be strong evidence that something has changed — a shift in a mean, a widening spread, or a steady march towards a limit.
That makes OOT the more valuable of the two, because it arrives earlier. By the time a result is OOS the batch is already a problem. An OOT signal detected three batches earlier is a process question with time to answer it. Sites that only monitor OOS have deliberately chosen the later signal.
The hard part is defining criteria. Purely statistical limits derived from a stable process are the sound approach, but they require enough data and a genuinely stable baseline. Fixed rules of thumb are easier and cruder. Whichever is used, the criteria should be documented in advance with a rationale, because criteria invented after an uncomfortable result are not criteria.
OOT matters most in stability studies, where a trend towards a limit predicts a shelf-life problem long before any result fails, and in continued process verification, where it is the mechanism by which the validated state is shown to be maintained rather than assumed.

Common failure modes
- No OOT procedure at all, so drift is only detected once a result fails.
- Trend limits set so wide that nothing ever triggers, which is indistinguishable from having none.
- Data collected for continued process verification but never interpreted — a documentation exercise, not a control.
- OOT signals raised and then closed without investigation because the result was in specification.
